When it comes to helping individuals transition into safe, stable housing, the process should be simple for both clients and referral partners. At Future Hope Solutions (FHS), we’ve designed a clear, stress-free referral system that ensures clients are placed quickly, affordably, and with the right fit.
This article outlines the exact steps case managers, discharge planners, and community agencies can take to connect clients with housing through FHS.
Step 1: Connect With Us
Referral partners can reach out via our website, phone, or email. Once we connect, we’ll provide information about current housing availability and criteria for each property.
Step 2: Review Program Details
Before referring, partners and clients can review our Program Details Page, which explains our housing model, house rules, payment expectations, and eligibility criteria. This ensures there are no misunderstandings.
Step 3: Submit Referral Information
We provide an easy referral form where partners can share client details, specific needs, and any supportive documentation required.
Step 4: Client Intake and Background Check
Once the referral is received, we schedule an intake to learn more about the client. A background check ($15 fee) is completed to ensure a safe environment for all residents.
Step 5: Housing Placement Decision
If the client qualifies and space is available, we match them to the most suitable property. If space is not available, we keep referral partners updated on future openings.
Step 6: Lease Agreement & Payment
Once accepted, clients review and sign the lease agreement. All payments (placement fees and rent) are due upfront to secure the spot.
Step 7: Move-In Orientation
Clients receive move-in documents, house rules, and personalized key codes for their room and entry. Our properties are move-in ready with utilities included.
Step 8: Ongoing Communication
Referral partners receive updates and can always reach out with questions. We maintain open communication to make the process seamless.
Q1: How quickly can a client move in after referral?
A: If space is available and the referral form is complete, clients can often move in within 3–5 business days.
Q2: What fees are required for clients?
A: Clients pay a non-refundable placement fee plus the first month’s rent upfront. Utilities are always included.
Q3: Can referral partners tour properties before sending clients?
A: Yes, partners are welcome to request property information or schedule a tour to better understand our housing model.
Q4: Do you provide case management or supportive services?
A: No, we are a housing provider only. We focus on providing safe, affordable homes while referral partners continue to provide services.
Q5: How do you ensure client safety?
A: All properties feature coded locks, cameras in common areas, and clear house rules. Every client undergoes a background check for community safety.
